HIPAA Notice of Privacy Practices (NPP)

This Notice describes how medical and health-related information about you may be used and disclosed by the CSM Cancer Foundation (“CSM,” “we,” “our,” or “us”) and how you can access this information. Please review it carefully.

Our Commitment to Your Privacy

The CSM Cancer Foundation is committed to protecting the privacy and confidentiality of health-related information shared with us by cancer patients, survivors, and caregivers. While we are not a healthcare provider, we voluntarily follow HIPAA-aligned privacy and security practices to safeguard Protected Health Information (PHI) entrusted to us through our programs and services.

What Is Protected Health Information (PHI)?

PHI includes any information that:

  • Relates to an individual’s health condition or treatment

  • Is provided in connection with assistance or support services

  • Can reasonably identify the individual

This may include medical details shared during applications, intake forms, or support requests.

How We May Use and Disclose PHI

We may use or disclose PHI only as permitted or required by law and only to support our mission. Uses may include:

  • Program Services: Determining eligibility and providing assistance, Care Cards, or financial support

  • Operations: Internal review, recordkeeping, and service improvement

  • Communications: Responding to inquiries or coordinating support

  • Legal Requirements: When required by federal or state law

PHI is never sold or used for marketing purposes.

Safeguards in Place

We protect PHI through:

  • Limited access to authorized personnel only

  • Secure digital and physical storage practices

  • Confidentiality agreements for staff and volunteers

  • Ongoing review of privacy and security practices

Your Rights Regarding PHI

You have the right to:

  • Access: Request access to your PHI

  • Correction: Request corrections to inaccurate information

  • Restrictions: Request limits on how your PHI is used or shared

  • Confidential Communications: Request communication through alternative methods

  • Copy of This Notice: Receive a paper or electronic copy of this Notice

Requests must be submitted in writing using the contact information below.

Our Responsibilities

We are required to:

  • Maintain the privacy and security of PHI

  • Provide this Notice of Privacy Practices

  • Follow the terms of this Notice currently in effect

  • Notify affected individuals in the event of a breach involving unsecured PHI, in accordance with applicable laws

Changes to This Notice

We reserve the right to change this Notice and make updated provisions effective for all PHI we maintain. Updated notices will be posted on our website with a revised effective date.
